Step 1: Download and Complete the
Off-Campus Application**

Complete an Application Form for each child applying to the FHCA Off-Campus Program.

A new form must be completed each year for each child enrolling in the program. Download a PDF of the form by clicking below OR use our online form** by clicking here.

**PLEASE NOTE: IF you use our online form, you can pay your tuition fees and upload student documents - for ALL students you are registering - in one convenient process.

We accept enrollment into our off-campus program year-round. If you have a new student who is a senior (12th grade), please contact us before completing application.

Step 2: Download and Complete the
Curriculum Form

Download and complete a Curriculum Form for each child joining the Program.

A new form must be completed each year for each child enrolling in the program.

If you haven't completed this step yet, please do so now.

Step 3: Download and Complete The Fee Payment Form**

Download and complete the Annual Off-Campus Tuition & Fees Form.

**PLEASE NOTE: If you use our online form to register your student(s), you may pay fees during the application process using credit card/debit/e-check!

Only ONE of these forms per family needs to be returned to FHCA.

Annual Achievement Testing
for FHCA off-campus students

Faith Heritage Christian Academy requires off-campus students to participate in annual achievement testing.  Achievement testing is required as part of our accreditation status as a school.  We understand that testing can be a source of many questions for parents and students, so we have some answers to frequently asked questions for you to review.  If you have a special need that you would like to speak with our staff about, please call or email our FHCA Off-Campus office at 901-872-0828.
